Audit observed that twelve (12) payment vouchers totalling N21,532,906.00, raised in 2017 has no relevant supporting documents such as originating memo, approval for payment, etc. attached. In the absence of these documents, audit found it difficult to accept those payments as legitimate charges against public funds.
The Executive Director is required to account for the sum of N21,532,906.00, failing which sanction stated in FR 3106 should apply.
