The sum of N6,000,000 (six million naira) was paid to the director (Administration) for the maintenance of leaking roof, construction of underground water facility and 2 tank strands, procurement of 4 units of air-conditioner and window blinds. The sum was released in two tranches of
i. Payment voucher No. 52 of Nov., ’19 – N3,000,000
ii. Payment voucher No. 2 of Dec., ’ 19-N3,000,000
Attached to the payment vouchers were invoices for the procurement and works done. It was discovered in audit that the roof claimed to have been maintained last year was still leaking. Furthermore, an invoice bearing serial number 0557 from OKE-TECH GLOBAL SERVICES of BA4/11 Timber Shed, Umuokpu Awka, and Anambra State was duplicated (in photocopies) and used to retire two different transactions. The details are;
i. On payment voucher No. 52 of Nov., 2019 for N3,000,000, a photocopy of the receipt was used to cover transactions on assorted paints valued at N250,000 (Two hundred and fifty thousand naira).
ii. Again, on payment voucher No. 2 of Dec., 2019 for N3,000,000, a photocopy of the same receipt was used to cover transactions on window blinds valued at N200,200 (Two hundred thousand naira).
The authenticity of these transactions could not be established in audit.
The director (Administration) was thus requested to show proofs that these transactions were genuine and explain why he had to fake an invoice to cover the transactions. In the alternative, the sum of N450,000 (Four hundred and fifty thousand naira), being the total of amounts he covered with a faked invoice, should be recovered from the director (Administration)
