Three (3) payment vouchers worth N14,995,000.00 paid to Chairman Vigilante group in respect of allowances and supplies of food items were not signed and attached with relevant supporting documents and evidence of channeling vide the stores accounting processes.
Failure to sign the payee portion and attached with relevant supporting particulars/documents to the payment vouchers could amount to the fact that the beneficiary(s) did not actually received the amount vouched for, and in such circumstances, the amount involved should be returned to account.
