On payment voucher No. 17 of December 2018, the sum N, 2187, 500 (Two million, one hundred and eighty-seven thousand, five hundred naira) was paid to a company identified as FUOBAJ Nig. LTD for renovation of works & health department buildings at Anambra East LGA, secretariat complex, Otuocha. Attached to the payment voucher was an invoice from EMMABROS Global Venture of No. 8 Old Mkt. Rd,. Nwko Nnewi with serial number 000046 and dated 17/12/2018.
It was observed in audit that on payment voucher No. 9 of December 2018 an invoice that shared striking similarities in all material particulars except in the amount had been used in retirement. The only difference was that the invoice bore N2,187,500 in one transaction and N1,707,500 in another transaction.
The impression created was that neither the transactions in payment voucher No. 17 nor 9 validly took place. Otherwise, why use the same invoice for different transactions of different values.
The permanent secretary was requested to obtain the explanations of the accountants on this obvious complicity in fraud. It was further requested that project sites inspection by the resident Auditors and facilitated by the Ministry should be arranged. Meanwhile, a report of job completion sighed by a qualified member of staff of the ministry was to be forwarded to me.
In the alternative, the sum of N3,895,500 (that is, N2,187,500+ N1,707,500) should be refunded to the public authorized the payment.
