On payment voucher No. 1 of November, 2020, the sum of six million naira (N6,000,000) only was paid to HOD, Art and Culture for 2020 Iri Ji (New Yam) Festival. Upon enquiries and study of the attached invoices and honour certificates, it was established that this festival was not held. The invoices and honour certificates were contrived. There were no evidence of when this festivals was held as the invoices and honour certificates contained dates were within the range of September and October, 2020 even when she received from the Head of Accounts and signed for the sum of N6, 000,000 on the 10th of November, 2020
The Permanent Secretary was requested to recover from (HOD, Art and Culture) the sum of N6,000,000 she received for a project not executed . She should pay the said sum back to Government treasury and forward the treasury receipt to me for further necessary action.
