Audit observed that:
i. The sum of 11,366,651.08 (Eleven million, three hundred and sixty-six thousand, six hundred and fifty-one naira, eight kobo) was paid for various items through six(6) payment vouchers, and
ii. The payment vouchers were raised more than three months before payments was made.
The Medical Director is required to:
i. Provide reasons why payment vouchers were allowed to stay for months before payments given that Internal Audit Unit shall not hold unto payment vouchers longer than 48 hours.
ii. Account for the sum of 11,366,651.08 (Eleven million, three hundred and sixtysix thousand, six hundred and fifty-one naira, eight kobo) expended on the supply of store items without appropriate documentation, and
iii. Remit the sum of 11,366,651.08 to the Treasury.
iv. Forward evidence of remittance to the Public Accounts Committees of the National Assembly, and
v. Otherwise, apply sanctions relating to irregular payment prescribed by paragraph 3106 of the Financial Regulations.
