Audit observed from the review of the Centre’s books of account that:
i. Payments (commissions) due to a firm between 2016 – 2019, totaling N22,445,561.43 were made to a staff’s Diamond Bank Account number 0052928995
ii. The staff is an employee of FMC KEFFI with the following details:
· IPPIS NO: 300529
· FILE NO: 2680
· DATE OF FIRST APPOINTMENT: 28/4/2014
· Records revealed that the above staff operated the underlisted accounts;
· Diamond account No. 0052928995
· GTB 0157698270
· Keystone 6014109180
· Access Bank 0785916950
iii. The Diamond Bank account number 0052928995 was formally bearing the staff
account name. However, it was observed to have been changed from the staff
name to the firm name, and
Iv. This was a pointer to the fact that the company was owned and run by the staff of
the FMC Keffi and the revenue collection function was outsourced to the staff of the centre.
