1. The sum of N558,007,192.09 (Five hundred and fifty-eight million, seven thousand, one hundred- and ninety-two-naira, nine kobo) was generated as revenue from 2016 to 2019 through the application of care plus software.
2.A consultant is the owner of the care plus software which the Centre used in collection of its revenue from 2016 to 2019.
3. Requests for access to care plus software in order to confirm the completeness of the transaction statement for the reported revenue of N558,007,192.09 were never attended to, and
4. Memorandum of Understanding (MoU) with the care plus software provider was not made available for audit examination.
1. Furnish explanations for the denial of access to revenue records.
2. Account for revenue of N558,007,192.09 generated through care plus software.
3. Recover and remit to the Treasury the sum of N558,007,192.09 being revenue generated from 2016 and 2019.
4. Forward evidence of remittance into the Federation Account to the Public Accounts Committees of the National Assembly, and
5. Otherwise, apply sanctions relating to failure to collect and account for Government Revenue specified in paragraph 3112 of the Financial Regulations
