The sum of seven million, ninety-eight thousand naira (N7,098,000) only was paid vide payment voucher number 23 of 14th of February, 2014 in favour of the Accountant, for construction of Dump Bay in Awka and Nnewi. Similarly, payment voucher number 35 of 21st March, 2014 for the sum of five hundred thousand naira (N500,000) only was paid to the Accountant for construction of Dump Bay for ASWAMA by bin. Lastly the sum of sum of two hundred and eighty thousand naira (N280,000) only was also paid to the same Accountant for sundry expenses vide payment voucher numbers 8 and 27 for N180,000 and N100,000 respectively. Audit could not be provided with any documented evidence for proper utilization of the above mentioned sums of money
As such, the accountant was requested to give proper account of N7,098,000, N500,000 and N280,000 paid to her as stated above and forward to me all relevant documents to support her claims. In the alternative, the amount should be recovered from her and all documented evidence of account/ or recovery forwarded accordingly for my confirmation and further necessary actions.
